D.O.T. recordable Accidents: None in 12 months

Ticket/Accident/Incident Combination: Limit 3 in 3 years
No pending or leaving accidents in the last 7 years
No license revocation past 3 years

Major Moving Violations: No serious in commercial vehicle in last 3 years.
